High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw steaks stored on top of raw fish and raw pork inside reach in drawers under grill station. Operator removed. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed a soiled slicer; per the operator, it was used just a short time ago.

Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed several containers with vegetables and marinated meats inside walk in cooler with no labels. As per operator were prepared in the morning.

Basic - Ceiling tile missing. Observed ceiling tile missing inside kitchen area.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tile soiled inside kitchen area near three compartment sink. Observed ceiling soiled inside walk in cooler.

Basic - Cutting board has cut marks and is no longer cleanable. Observed Multiple white cutting boards with knife grooves inside kitchen area and prep area.

Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Coached operator.

Basic - Equipment in poor repair. Observed a wooden 2x4 being used to support a shelf inside the walk-in cooler.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave handle and inside soiled with some grease located on top of prep table.
