Basic - Reach-in freezer interior/shelves have accumulation of soil residues. Observed hamburger reach in freezer near cook line soiled with old food debris.

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed cook placed raw beef patty on grill and then changed gloves and began handling bread without washing hands.

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef stored over griddle cakes inside reach in freezer at cook line. Manager rearranged products. Corrected On-Site
