High Priority - Dented/rusted cans present. Observed a dented can of pizza sauce by Papa Johns at 6lbs 11oz and pineapple tidbits by Dole at 6lbs 10oz, stored on rolling rack at back kitchen area. Manager removed canned food items from rolling rack. Corrective Action Taken

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust accumulation on air vent located above stacked pizza boxes on shelving near screen monitor.

Basic - Exterior door has a gap at the threshold that opens to the outside located at back kitchen exit door. Repeat Violation

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed the can opener blade soiled.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed a hole in the ceiling across from the water fountain.

Basic - Equipment in poor repair. Observed the prep reach in cooler gaskets torn.

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed at the back door.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed several handles for cheese and other pizza toppings in the preparation cooler.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the walk-in cooler fan guard soiled.
