Basic - Ice buildup in reach-in freezer next to the fryers.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler door gaskets soiled at the front counter.

Basic - Working containers of food removed from original container not identified by common name. Observed a container with sugar not labeled at the front line. Manager labeled it. Corrected On-Site

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles ext to office sagging.

Basic - Equipment in poor repair. Observed gasket torn at reach in freezer located at front line across from fryers. Observed gasket torn at ice machine located at kitchen area.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall under coffee station soiled with debris.

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee at cook line wearing a watch while preparing food.

Basic - Open dumpster lid. Observed dumpster lid open manager, had employee clothes during inspection. Corrected On-Site

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee wiped preparation table with the wiping cloth, dried her hands with paper towel and also her apron to continued to prepare food without washing her hands and change her gloves. Coached manager on proper hand washing procedures. Corrective Action Taken

Basic - Reach-in freezer interior have accumulation of soil residues, freezer located in front of the fryers.

High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed raw chicken over precooked chicken fried. Operator moved the products. Corrected On-Site

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Basic - Exterior door has a gap at the threshold that opens to the outside.

Basic - Grease on the ground and/or pad around grease receptacle.

Basic - Light shield damaged/in disrepair. Observed no light shield at walk in freezer.

Basic - No conspicuously located ambient air temperature thermometer in walk in cooler and walk in freezer.
