Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook cracking raw unpasteurized shell eggs and touched clean plate without changing gloves and washing hands. Operator had employee place soiled dish in dishwasher area. Operator explained proper protocol for Handwashing and glove use. Employee washed hands and put on clean gloves. Observed Corrected On-Site
