Intermediate - Handwash sink used for purposes other than handwashing. Observed ice and soda remnants inside hand wash sink at server area. Instructed manager and employees that handwashing sinks are to be used for handwashing only.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine located near the rear kitchen exterior door.
Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile in disrepair above beer cooler at server station.
Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on prep table next to pizza oven. Operator removed beverage and properly stored. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed build up of grease on hood filters above pizza oven. Observed water heater surface soiled and rusted. Observed air conditioning vent soiled with dust located above prep table near pizza oven.
Basic - Objectionable odors in bathroom or other areas of the establishment. Observed by smelling objectionable odor in mop sink room. Operator made plans with employees to do a deep clean of the mop sink room. Corrective Action Taken
