Intermediate - Interference/obstruction with the inspector's right of entry and access at any reasonable time. Inspectors entered through open front entrance of hall and approached kitchen. Inspectors approached operator after identifying selves. Operator stated she will not let us do the inspection and that we are required to call her in advance to schedule. Operator states she is not working today, when asked by inspectors about the food sitting out on the table, she then stated they are just finishing up and leaving. Operator stated again that we are not allowed to be there and we have to leave, she also stated that she will be filing a complaint against us. Inspector made operator verbally aware that denying access could lead to an Administrative Complaint. Operator stated that is fine and instructed us to leave again. Operator had multiple food items on prep table. There were multiple pans of cooked comminuted meats on the counter and multiple covered pans. Operator also had one employee on duty. Admin Complaint

Basic - Equipment in poor repair. High temp dish machine rinse at 155F after 3 cycles. Operator will use 3 compartment sink to sanitize with quaternary ammonium sanitizer. Operator was not using the machine when we arrived.
