Intermediate - No soap provided at handwash sink. Observed no soap at hand washing station near entrance to building. Discussed with person in charge.

Intermediate - Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water shut off to hand washing station near entrance to building.

Basic - Bathroom facility in disrepair. Observed employee restroom toilet missing toilet seat.

Basic - Garbage can located outside has no lid or lid open/broken. Observed two municipal garbage cans missing lids creating a sanitary nuisance by attracting vermin and allowing odors to escape can. Warning

Basic - - From initial inspection : Basic - Plumbing system in disrepair. Observed plumbing to hand washing sink not connected to anything. Observed dish washer plumbing dumping into triple sink. Warning - From follow-up inspection 2025-10-24: Observed plumbing to hand washing station outside connected and no longer draining onto ground. Observed dish machine drain hose still hanging in triple sink. Time Extended

High Priority - Sewage on ground from failed septic system. Observed waste water and sewage pooling on property behind food truck near a utility box with a red light. Utility box with light has switch in "silence" mode per label on side of box. Admin Complaint
