High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Upon inspection observed peanut butter yogurt mixture in reach prep cooler right side of service window in kitchen without date mark present. Operator stated it was prepared 3 days ago.
