High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed vacuum breaker missing from non chemical side of splitter at mop sink.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed slicer in prep area have accumulation of dried debris.

Intermediate - Handwash sink used for purposes other than handwashing. Observed hand wash sink on cook line used to store large water pitcher. Employee removed pitcher from hand wash sink. Corrected On-Site Repeat Violation

Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed paper towel dispenser next to triple sink unable to dispense paper towels. Operator removed paper towels from dispenser. Corrected On-Site

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.
