Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple Reach-in-cooler drawer and door gaskets have build up of food debris. Operator assigned employee and cleaning began. Corrective action taken

High Priority - Employee washed hands with cold water. Observed a employee wash hands with cold water after handling raw shell eggs. Educated on the importance of washing hands with hot water at a minimum of 100F.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no Vacuum Breaker at the hose bib in the trash receptacle area.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance on the self surface drink machine. Operator washed,rinsed and Sanitized the ice chute. Corrected On-Site

Basic - Clean equipment/dishware/utensils stored next to handwash/food preparation sink exposed to splash. Observed food trays next to the hand wash sink on the main line. Operator moved the trays. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed excessive ice build up in the walk in freezer. This is a repeat violation from 05/14/21, and 09/10/21 discussed with operator about a service call to repair the pipe in the walk in freezer. Repeat Violation

Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed a employee drink in the grill cooler. Operator removed the drink. Corrected On-Site

High Priority - Toxic substance/chemical improperly stored. Observed a mosquito spray by the hand wash sink on the main line. Operator removed the mosquito spray. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance on the soda nozzles at the self dispensing soda station. Operator washed, rinsed and sanitized the soda nozzles. Corrected On-Site

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water at 90°F at the hand wash sink in the employee ladies room after being ran for five minutes. This is a repeat violation from 05/14/21. Repeat Violation

Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at the hand wash sink on the front line. Operator supplied the hand wash sign.
