Basic - In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed at pizza station - scoopers stored in shredded cheese container. Employee removed and stored correctly. Corrected On-Site

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at flip top cooler - tomato sauce (56F - Cold Holding) as per operator item held out of temperature for approximately 30 minutes - item not portioned or prepared today - operator placed bags of ice to bring temperature down. Corrective Action Taken

Intermediate - Soap dispenser at handwash sink not working/unable to dispense soap. Next to pizza station flip top.

High Priority - Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. At kitchen station - Observed employee taking soiled utensils to the triple sink, then go to prep area and touch clean and sanitized utensils, without hand washing. Educate operator of proper hand washing procedures. Corrective Action Taken

High Priority - Toxic substance/chemical improperly stored. Observed cleaning solution stored above; clean food utensils. Operator removed. Corrected On-Site

Intermediate - Certified Food Manager or person in charge lacks knowledge of employee health policy and health requirements. Educated and provided operator with the health policy vouchers. Corrective Action Taken
