High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink, in the mechanical room in the lobby hallway.

Intermediate - Lack of toilet tissue at each toilet. Observed : in the women's restroom, lack of toilet paper in the far stall.

Intermediate - No soap provided at handwash sink. Observed : the soap dispenser at the hand wash sink in the kitchen would not dispense soap, it was dried in place soap. Dish soap was used to wash hands.
