NO CONSUMER ADVISORY PROVIDED ON MENU FOR UNDERCOOKED BURGERS ON MENU. A CONSUMER ADVISORY MUST BE PROVIDED FOR ALL UNDERCOOKED MEATS EXCLUDING WHOLE MUSCLE INTACT BEEF STEAKS. MANAGER INFORMED OF REQUIREMENT AND SHOWN SECTION OF RULE SHOWING THE PHRASING TO INCLUDE.

OBSERVED ROP STEAKS AND SALMON THAWING IN A PREP SINK WITHOUT BEING REMOVED FROM ROP PACKAGING. ANY ITEM WHICH IS IN ROP MUST BE THAWED AS INDICATED BY THE MANUFACTURER AND REMOVED FROM THE ROP PACKAGING TO PREVENT POTENTIAL BOTULISM GROWTH. MANAGEMENT EDUCATED ON ROP PROCEDURES AND THE IMPORANCE OF CHECKING AND FOLLOWING MANUFACTURER INSTRUCTIONS AS WRITTEN ON BOX.

OBSERVED STAFF HANDLING SEVERAL TASKS WITH THE SAME GLOVE ON. GLOVES SHOULD BE USED FOR ONE TASK ONLY, HANDS SHOULD BE WASHED, AND NEW GLOVES PLACED BEFORE MOVING TO A NEW TASK. STAFF EDUCATED ON PROPER GLOVE USE. ENSURE THAT TRAINING IS REINFORCED FOR PROPER GLOVE USAGE TO PREVENT CROSS-CONTAMINATION.
17C - physical facilities installed, maintained, and clean

OBSERVED THAT A NEW SECTION OF THE BASEMENT HAS BEEN MODIFIED FOR USE OF FOOD SERVICE OPERATIONS. THIS NEW AREA HAS NOT BEEN APPROVED BY THE HEALTH AUTHORITY FOR FOOD SERVICE USE. ALL FOOD SERVICE OPERATIONS MUST CEASE IN THE NEW AREA UNTIL A PLAN REVIEW IS SUBMITTED AND APPROVED, AND THE AREA MEETS COMPLIANCE WITH FOOD CODE. THE NEW AREA HAS EXPOSED RAW WOOD CEILINGS, WHICH IS NOT A CLEANABLE SURFACE.
17D - adequate ventilation and lighting; designated areas used

OBSERVED EMPLOYEE DRINKS IN COOLERS WITH FOOD SERVICE ITEMS AND PLACED ABOVE WORK STATIONS. ALL EMPLOYEE FOOD ITEMS SHOULD BE CONTAINED IN AN EMPLOYEE-SPECIFIC AREA, AND ALL DRINKS AND PERSONAL ITEMS MUST BE PLACED BELOW THE WORK SURFACES TO PREVENT CONTAMINATION. ITEMS MOVED TO APPROPRIATE EMPLOYEE FRIDGE.
