Observed rice in crock with a temperature of 95 degrees Fahrenheit. All hot holding shall be held at 135 degrees Fahrenheit or higher. Rice was discarded.

Observed several foods and food products not labeled. All foods and food products removed from its original container shall be labeled with the food/food products common name
12A - contamination prevented during food preparation, storage, display
Observed business phone stored on cutting board in food prep area. Phones, personal belongings shall be stored away from food prep area.

Observed plastic spoons being used for scoops in seasoning containers. Proper scoops shall be supplied and ensure handles are place in an upright position to prevent contamination.
