Observed bulk container of coleslaw in walk-in cooler without a date marker. CA: refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. COS: Coleslaw was discarded.

Observed equipment in poor repair. Gaskets/seals on coolers need to be repaired an maintain cleanliness. Sandwich prep top cooler in need of prep top door repair. CA: Discussed with PIC importance of keeping equipment in good working condition.

Observed several areas of facility in need of repair. Floor tiles and tiles around baseboard missing or broken and cracked. Back door has a gap in the bottom. CA: PIC will place repair order request to have items of physical facility repaired properly.

Observed time/temperature control for safety (TCS) food items stored above 41 degrees Fahrenheit in reach-in cooler (milk- 44F; cut lettuce- 50F; orange juice- 43F; see temperature chart). CA: PIC stated that items had been held above 41F for longer than 4 hours. PIC discarded items. <b>Cooler must be serviced and rechecked by Health Department prior to bringing back into use.</b>

Observed broken shelving in walk-in freezer. Observed floors and walls in walk-in freezer and around food prep areas in need of cleaning. CA: Discussed increasing cleaning frequency of floors and walls, repairing broken shelving.
