Observed raw shell eggs stored above ready-to-eat food items in WIC. COS: PIC moved raw shelled eggs to shelf below ready-to-eat food items in WIC.

Observed personal food items stored above and adjacent to food for service in WIC. CTM: Rearrange WIC shelving to protect food for service.

Observed evidence of insufficient pest control methods - presence of trapped insect pests and harborage conditions (personal items stored at facility, unnecessary equipment stored, cleanliness issues (see violation above). Pest control completed through Astro - records indicate last visits on 3/16/2026, 2/28/2026, 1/19/2026 Complete cleaning and removal of extraneous items and have Astro give recommendations
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed rice scoops stored in water not at 135 degrees Fahrenheit or above (measured at 91 degrees Fahrenheit) COS: PIC took the scoops to be washed, rinsed and sanitized before being placed in water 135 degrees Fahrenheit and above - utensils and container must be cleaned every 24 hours.

Observed nonfood-contact surfaces of food prep equipment in kitchen with accumulated food debris. CTM: Clean these areas until they are free from food accumulations - prep table, slicer, dish storage area, between fryer and other equipment
