3-1C - food in good condition, safe, and unadulterated
Regulation: 511-6-1.04(1) - safe, unadulterated and honestly presented (p)
Observed food (box of lemons) with mold-like growth and rotten in walk-in cooler. CA: All food shall be in good condition. COS: Lemons discarded.
1-2A - pic present, demonstrates knowledge, performs duties
Regulation: 511-6-1.03(2)(a)-(n)(p),(q) - responsibility of pic (pf)
Observed person in charge (PIC) not performing duties; not ensuring delivery drivers are taking 3 food temperatures and noting on invoice for key drop deliveries, not having completed employee reporting agreements, not ensure dishes are cleaned and sanitized, not ensuring food is in good condition. CA: PIC shall be properly trained on food safety protocols for establishment. Refer to rules for a complete list of required duties. COS: Re-inspection will be conducted within 10 days for reassessment.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed PIC not providing documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Every employee shall have a signed copy of the employee reporting agreement available for viewing either hard copy or electronically. COS: Instructed to review roster of employees to ensure all employees have a signed copy of employee reporting agreement.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed old food stuck to clean dishes. CA: All dishes shall be clean to sight and touch. COS: Dishes placed in dish washing area to be rewashed. Train employees to check all sides of dishes to ensure no old food.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed non food contact surfaces in need of cleaning: countertops in both microwave areas, under grill, bump bar, sticker holder, wing warmers, inside of hot holding units, hand wash sinks in kitchen. CA: Establishment shall be cleaned as needed to prevent the buildup of food debris. Perform deep clean of kitchen.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors in entire kitchen under all equipment in need of cleaning. Coated with grease and old food. Observed floors in walk-in freezer in need of cleaning. CA: Floors shall be cleaned under equipment and in walk-in freezer as needed to prevent the buildup of trash, grease and food.
