Cold-Holding was assessed and incompliance unless noted in the temperature log. Hot-Holding was assessed and incompliance unless note in the temperature log. Gave the following to PIC; 1. (2) employee health forms 2. employee health policy 3. written procedures for a vomiting and diarrheal 4. written time control procedures (6 hours) and (4 hours) Ensure a sneeze guard is placed over preserves. Suggested using time control for foods that cannot be kept 41F or below in the self serving area. Informal required by 2/27/26 ***FAILURE TO HAVE SIGNED EMPLOYEE HEALTH FORMS MAY RESULT IN PERMIT SUSPENSION. PERMIT WILL BE REINSTATED WHEN SIGNED FORMS ARE PROVIDED*** Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissible through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. (Pf) Corrective Actions: PIC was given an employee health policy and coached on reportable diseases.

Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. 2-2E: .03(6)Facility was unable to provide written procedures for a vomiting and diarrheal event. A food establishment shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. (Pf) Corrective Actions: PIC was given written procedures for a vomiting and diarrheal event.

6-1A: .04(6)(f)Observed boiled eggs, skim milk, and 2% milk cold holding above 41F in the serving area. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P) Corrective Actions: PIC discard the skim milk and 2% Milk. Boiled eggs were place in an ice bath.

10D: .04(8)(b)Observed three containers of skim milk with an expiration date of 2/11/26. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as "keep refrigerated" and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer's expiration date or the sell-by date. (P) Corrective Actions: PIC discarded the milks.

12A: .04(4)(u)Observed multiple apples in the self serving area not covered. All food, whether on display, being prepared for service or placed for consumer self-service shall be protected from contamination from consumers standing or sitting within eight feet of the food. (P) Corrective Actions: PIC wrapped the apples.
