Cold-Holding was assessed and incompliance unless noted in the temperature log. Gave 2026 permit to PIC. Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Repeat violations may result in permit suspension. 1-2B: .03(3)(c)Observed the CFSM ( certified food safety manager) document blocked by tall multiple stacks of pizza boxes. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. (Pf) Corrective Actions: PIC (person in charge) removed the pizza boxes to an approved location

2-2B: .03(5)(j)1&2Observed personal drinks stored on the food contact table located across the oven. Observed another personal drink stored on the prep table located by the hand sink in the warewashing area. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). (C) Corrective Actions: PIC removed all the drinks to an approved location.

2-2D: .06(2)(o)Observed the hand sink located in the warewashing area blocked by a large red trash can. A handwashing sink shall be maintained so that it is accessible at all times for employee use. (Pf) Corrective Actions: The red trash can was removed to an approve location

12B: .03(5)(g)Observed food employee prepping pizza dough while wearing a bracelet. Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band). (C) Corrective Actions: Food employee stated she could not remove the bracelet without cutting it. Food employee then put on gloves and continued to handle pizza dough.

13A: .02(1)(d)Observed the inspection report blocked by multiple stacks of pizza boxes. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. (C) Corrective Actions: PIC removed the pizza boxes to an approved location.
