Please reach out to our department if facility needs permit, inspection report, and any other documents. Facility could also go on our website for documents such as employee health forms and sample key drop agree. Complaint investigation closed. Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Repeat violations may result in permit suspension.
