1-2B: .03(3)(c)Observed the CFSM (certified food safety manager) document not posted into public view. PIC pointed to a glass board containing all of the facility CFSM stored on the floor behind the cahsier area. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. (Pf) Corrective Actions: PIC placed the glass board on the counter.

2-2A: .03(2)(o)Observed PIC was unable to provide signed employee health forms. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: THIS IS THE SECOND REPEAT. OBSERVATION OF VIOLATION ON NEXT ROUTINE INSPECTION WILL RESULT IN PERMIT SUSPENSION ***FAILURE TO PROVIDE SIGNED EMPLOYEE HEALTH FORMS MAY RESULT IN PERMIT SUSPENSION. Food must be protected from cross contamination by storing the food in packages, covered containers, or wrappings, except for loosely covered, or uncovered containers in which food is being cooled if protected from overhead contamination. (C) Corrective Actions: PIC covered the containers of butter and the container of penne noodles.

13A: .02(1)(d)Observed the previous inspection report not posted into public view. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. (C) Corrective Actions: PIC was given the current inspection report to post into public view.

13A: .02(1)(f)(1)Observed the 2026 permit not posted into public view. The permit holder shall post the permit as in a location in the food service establishment that is conspicuous to consumers. (C) Corrective Actions: PIC was provided the 2026 permit to post.
