SECOND REPEAT OF CODE VIOLATION 2-2D..03(2)(o) (HAND SINK BLOCKED) AND 13A.02(1)(D)(PREVIOUS INSPECTION REPORT NOT POSTED). OBSERVATION OF BOTH CODES ON NEXT ROUTINE INSPECTION WILL RESULT IN PERMIT SUSPENSION. Ensure utensils are not stacked wet. Informal on 4/23/26. Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Repeat violations may result in permit suspension. 2-2A: .03(2)(o)Observed PIC (person in charge) was unable to provide signed employee health forms for selected employees. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: PIC was given two employee health forms. Both employees signed the forms.

A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer's instructions. (Pf) Corrective Actions: Trash can was rolled to an approved located. SECOND REPEAT OF CODE VIOLATION. 2-2E: .03(6)PIC was unable to provide written procedures for a vomiting and diarrheal event. A food establishment shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. (Pf) Corrective Actions: PIC was given written procedures for a vomiting and diarrheal event. Facility will use quats as disinfectant sanitizer.

The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. 17C: .07(5)(a),(b)1,2,3Observed flooring in the walk-in-cooler and underneath equipment in the serving line contained debris. Observed the left side wall in by the grill held grease debris. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)
