511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

1. Routinely inspecting incoming shipments of food and supplies;

2. Routinely inspecting the premises for evidence of pests;

3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

4. Eliminating harborage conditions. Note: Observed dead insects on ground inside womens RR. PIC to clean up dead inspects and routinely check for/ clean up.

511-6-1.06(5)(h) - Toilet Room Receptacle, Covered (C) (h) Toilet Room Receptacle, Covered. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Note: Observed employee RR trash can to be missing a lid. PIC to purchase a covered trashcan.

Hand washing sink on ice cream side of kitchen is missing a mixing valve. Water temperature was at 130 Degrees F. Told GC to install a mixing valve before leaving the store.

The bathroom in the kitchen for employees is missing the self closing door mechanism. GC was told to tell the owners to install/replace the missing closing mechanism.

The employee bathroom in the kitchen has a trash can but is missing the lid for the trash can. I told the GC to tell the owners to install a trash can lid.

Place an "Employees Must Wash Hands" sign in Men's and Women's restrooms that is used by employees.

Significant leak from drain line at hot water heater. Pooling water is causing a slip hazard, along with a possible electrical hazard with the presence of water. Repair immediately.

Multiple items in walk in cooler not labeled with common name nor date prepared.
