6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed the freezer in the back of the facility no longer holding freezer temps and the food inside including multiple roasts of meet and sausage and burger patties sitting at 44 degrees fahrenheit. CA: Time temperature control for safety food must be maintained at 41 degrees Fahrenheit or below for safety. COS: Food that was over was voluntarily discarded. Boxes that were on the floor soiled were discarded. EHS will need to verify freezer in functional before use of it can be continued.
1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(c) - certification documentation (pf)
No CFSM certificate on site in public view or for Health authority to view. CA: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times and shall be made available for inspection by the Health Authority. CFSM is clos to expiration. Print and place current one on the wall in public view and seek opportunities to renew certification.
12A - contamination prevented during food preparation, storage, display

Box of dinner rolls observed in the Freezer sitting on the floor with the box soaked in some juice or liquid. CA: 1.Except as specified in paragraphs 2. and 3. of this subsection, food shall be protected from contamination by storing the food: (i)In a clean, dry location; (ii)Where it is not exposed to splash, dust, or other contamination; and (iii)At least 6 inches (15 cm) above the floor. 2.Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts. 3. Pressurized beverage containers, cased food in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture. COS: Box discarded.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed many flies in the facility with no outside fly traps not strips on the interior away from food area to attempt to mitigate or eliminate the issue. Seal in the bag door has a crack that allows for the passage of flies in and out. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule;Pf and eliminating harborage conditions. Correct within 24-48 hours.
