Observed multiple food items in the prep top cooler at the pizza station holding at temperatures above 41 degrees F. TCS food being cold held shall be maintained at a temperature of 41 degrees F or below. COS: Food that was reading out had only been setting out for less than an hour and read at 41 degrees prior to coming out of the walk in cooler, they were rotated out for fresh product and cooled down to below 41 degrees prior to reserving. Ice placed in cold well of the PTC to help hold items till maintenance can assist

No CFSM certificate displayed in public view or retained on file to be made available to Health Authority upon inspection. A restaurant shall have in it employ a CFSM and the original certificate for their certification shall be displayed in public view and a copy retained on file to be available to Health Authority. CA: CFSM employed according to PIC but no certification is diplayed or on site. Display CFSM certification by next day of business.

Employee drink and food sittin out and open on the food prep counter in main kitchen area and the shelf in the the walk-in cooler, rather than in the designated area for it in the walk-in. Employees may have a drink in a cup with a LID and STRAW in a designated area away from food prep and storage. COS Employee discarded open containers.
Observed chicken wings marked with wrong date in prep cooler, marked 10/2 which was the prep date not the discard date. Date marking for this facility has everything labeled with discard dates not prep dates. 10/2 was verified to be the prep date not the discard date by GM of store. COS Employee put correct discard date on items.

Observed personal items (backpack and speaker) from employees sitting on top of dry storage good in the back of the restaurant rather than in the lockers provided. Personal items shall be stored in designated area for employees not in food prep or storage areas. COS Employee moved items to proper place.
