Regulation: 511-6-2-.13(1) - insect and rodent control
Observed roach on wall in room 106.
CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises.
Confirmed with PIC that facility has pest control contract. Verified last date of service was 5/11/2026. PIC stated they are due for their next service soon and advised that he will have pest control company come back out and service the facility
9 - food offered in compliance with tourist accommodation permit or food service permit; appropriate bed and breakfast or continental breakfast operations and menu; proper food temperatures; handwashing and no bare hand contact
Observed tub of vanilla yogurt, garlic herb cream cheese, gallons of milk and orange juice in cooler for communal continental breakfast use.
CA: If no food service permit is available, potentially hazardous foods commercially must be prepared and packaged in single servings
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Observed dressers in rooms 102, 106, 115 (2/3 drawers taped shut) and 116 not in good repair./Observed microwaves in rooms 102, 106, 115 and 116 interior ceilings with dried food debris.
CA: All furniture, draperies, appliances, carpets, and other accessories, in the establishment must be maintained in good repair, clean, and free of vermin.
PIC stated they will be replaced soon/PIC will have microwaves cleaned
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair
Regulation: 511-6-2-.08(4) - toilet facilities-cleaning and maintenance
Observed colored ring in toilet in room 101./ Observed toilet seat in room 115 no longer in good repair and easily cleanable./ Observed walls above shower walls in rooms 106 and 115 with evidence of a past leak.
CA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair.
PIC will have toilet cleaned./PIC will have toilet seat replaced/ PIC will ensure walls are in good repair
17 - proper ventilation; no sign of mold, mildew, odors or moisture
Observed restroom exhaust fans in rooms 102, 106, 115 (fan not working) and 116 with an excessive buildup of dust and debris
CA: The current ventilation must be kept clean and prevent odor, mold, mildew, and moisture
