6-1A - proper cold holding temperatures

Food in prep top cooler was not being held at 41 degrees or below. Cooler did not appear to be working correctly despite thermometer reading saying otherwise. Restaurant removed food and will repair or replace unit.
4-2A - food stored covered

Various food items in walk-in cooler were found to be open with no protective covering to prevent contamination.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed flour, corn starch and sugar not labeled outside of its original container. Porducts not stored in original containers shall be labeled with its common name.
