Observed dumpster door open. CA: Dumpster door must be closed at all times. COS.

Observed clean containers wet stacked. Must air dry before stacking. Shall be air-dried or used after adequate draining before contact with food; and may not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry. COS: All items were re-washed and dried properly.

Observed ceiling vents soiled with dust and debris. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Observed back door left open. CA: Door must be left closed at all times when not in use.

Observed grease and dirt accumulated on the kitchen floor, vents, hood, around the equipment. Must thoroughly clean.

Observed multiple puddles of standing water throughout kitchen. CA: All floors must be able to properly drain, and there should be no standing puddles during service.

Observed Walk-In Cooler with an ambient air temperature of 44, and several TCS Foods (tomatoes, cheese, and raw chicken) temping at 43-45 degrees Fahrenheit. CA: Door sweep is currently in disrepair and the cooler is not able to hold proper temperature. Door sweep needs to be replaced and all cold-held food must be kept at or below 41 degrees Fahrenheit.

Observed back door left open. CA: Door must be left closed at all times when not in use. Door is currently difficult to open when closed. Inspector recommended that staff use doors in lobby to enter and exit the restaurant until back door is fixed.

Observed many living and dead flies throughout kitchen and dry storage hall. CA: Keep doors closed to keep flies out, and sweep up any dead insects as you find them. Recommend contacting a certified pest control operator for fly control devices to get flies under control.

Observed walk-in door with torn floor sweep that leaves a large gap on bottom of door. All doors to cold-holding equipment must have the necessary gaskets/floor sweeps in proper condition to keep units at required temperatures. The torn door sweep is also allowing standing water from leaking mop sink to get into the walk in cooler. CA: Door sweep must be replaced and door should be properly sealed when fully closed.

Observed multiple puddles of standing water throughout kitchen; one particular puddle under the coke machine by the drive-thru window was creating a terrible smell. CA: All floors must be able to properly drain, and there should be no standing puddles during service.

Observed incorrect ceiling tile type above 3-compartment sink and several other tiles with holes or not fitted tightly in ceiling. CA: Replace porous ceiling tile with one that is smooth, durable, and non-absorbent. Replace other necessary tiles or fill holes.
