Regulation: 511-6-2-.13(1) - insect and rodent control
EHS observed dead roaches in rooms 116, 123, and 209.
511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents.
C/A: PIC was informed that further measures will need to be taken in order to eradicate pests and dead roaches need to be cleaned
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
Regulation: 511-6-2-.14(6) - bed, bedding, and linens
EHS observed stains and/or tears on mattresses and bed linens in all rooms inspected.
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
C/A: PIC was informed that all bedding items that are in disrepair will need to be washed, refurbished, or replaced

EHS observed walkboards on second floor in disrepair.
511-6-2-.13(3) - Insect and Rodent Control-Openings Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks.
C/A: PIC was informed that the walk boards will need to be repaired

Floorboards in room 203 in were dislodged, showing gaps.
511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored.
