Observed container of cooked chicken below raw shrimp in reach in cooler in main kitchen. CA: Foods must be stored by minimum cook temperature. COS: No contamination was seen. Chicken was placed above shrimp.

Observed egg yolks stored above site made sauces in prep top underneath cooler. CA: Foods must be stored by minimum cook temperature. COS: No contamination was seen. Sauces were stored above egg yolks.

Observed avocados and container of celery directly touching raw beef in reach in cooler in front food service area. CA: Foods must be stored by minimum cook temperature. COS: PIC discarded foods.

Observed hard boiled eggs previously cooked stored wrapped and cooling in reach in cooler. CA: Food should be cooled in approved methods and equipment. COS: Eggs were placed in walk in cooler to properly cool.

Observed hot and sour soup previously cooked cooling in prep top cooler underneath area. CA: Foods should be cooled in approved cooling methods and equipment.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(l) - removing dead or trapped birds, insects, rodents, and other pest (c)
Observed heavy accumulation of insects on insect device in back of main kitchen. CA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
UPDATE: Site visit made and insect device was removed.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed multiple clean dishes (cups) wet stacked in drink alley. CA: After cleaning and sanitizing, equipment and utensils should be air-dried or used after adequate draining before contact with food. COS: PIC rewashed and restacked cups.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed phone and phone charger stored on and next to food contact surface next to sushi cooler area in front food service area. CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC removed items and relocated to designated area. Update: Site visit made on 3/5/26 and insect device was removed and being replaced with new one.
