Observed bottle of sanitizer stored hanging on wire shelving above parmesan cheese. CA: Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles. COS: PIC chose to discard the bottle of sanitizer.

Regulation: 511-6-1.10(5)(c) - conformance with approved procedures (p, pf)
Observed facility not abiding by approved variance. Pizza sauce as not dated nor had time when removed from cooler. CA: Facility utilizing variance must follow variance as approved by state. COS: Pizza sauce discarded.

Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed cigarettes on cart next to back door. CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC to move items to designated area.
