Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed container of diced tomatoes held in facility past 7 day discard date of 12/9, and multiple containers of diced tomatoes, container of coleslaw, and container of diced cucumbers held in faciliy greater than 24 hours without datemark label. CA: RTE TCS food items to bare date mark labeled to indicate food item to be sold, served, or discarded within 7 days, prep or opened date begins as day 1. COS: PIC discarded food items.
11C - approved thawing methods used

Observed sealed containers of meat thawing in container of still water in prep sink. CA: If thawing under water, water must be at a temperature less than 70F & at a sufficient water velocity to agitate and float off loose particles in an overflow. COS: PIC turned water on to have consistent flow.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed daylight coming thru bottom of back entry way door. CA: Outer openings shall be protected from outer openings of a food service establishment shall be protected against the entry of insects and rodents by ensuring doors are solid, self-closing, and tight-fitting. PIC to have door seal repaired by correct by date.
