Observed multiple open boxes of single use clam shells, deli papers, etc., with large debris and dust on the items. Debris is from recent ceiling tile repair. COS: All open boxes on this shelf were discarded by staff.

Observed heavy ice build up in two reach in freezers and mild ice build up starting in walk in freezer. CA: Maintain freezers free of ice accumulation and in good working order.

Observed heavy dust accumulation on slushie machine fan and creamer machine fan in front left prep area. Observed build up of splatters and dust on nonfood contact surfaces of equipment. CA: Increase cleaning frequency of nonfood contact surfaces of equipment to prevent build up.
