Observed uncovered/out of bag tilapia filets in box in reach in freezer in back storage room. COS: All items were properly covered. Rule: Food shall be protected from contamination by storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.

Observed container of butter, bowls of salt and pepper uncovered on counters. Observed lettuce without wrapping in the prep/reach in cooler (discarded). Observed bags of flour, sugar, other grains stored with bags cut open and not protected in any way in dry storage. CA/Rule: All food, except for when cooling in a protected area, must be stored in covered containers or wrappings to prevent contamination. Inspect and repackage the above mentioned items.

Observed house made kimchi with a prep date of 4/15 and chicken salad with a prep date of 4/28 in the tall reach in cooler. COS: Kimchi was discarded. Chicken salad was actually made on 5/28 and a new date label was placed on container. Rule: Refrigerated, ready to eat, TCS foods prepped in the facility AND manufactured TCS foods (such as milk) must be dated as the prep day or the day the container of manufactured food was opened. These items must be served or discarded within seven days. Day one is the prep/open day.

Observed reduced oxygen packaged tuna completely thawed in unopened package in the prep/reach in cooler. COS: Tuna was discarded. Rule: Reduced Oxygen Packaging(ROP) fish that bears a label indicating that it is to be kept frozen must be removed from the reduced oxygen environment prior to its thawing and/or immediately upon completing its thawing, using procedures of completely submerging in cold running water.

Observed dust/crumb/debris build up on shelves above prep tables, wire shelves, window seal behind 3 compartment sink, box fan, mixer equipment shelf, side of ovens(grease), plate and bowl shelf. CA/Rule: Nonfood contact surfaces must be cleaned frequently enough to prevent accumulations.
