Observed spray bottles stored on shelf above three compartment sink containing chemicals without labels. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. PIC needs to label spray bottles.

Observed multiple water damaged ceiling tiles near bathroom areas and in kitchen. Observed missing/broken ceiling tiles in bathroom area. All physical facilities shall be maintained in good repair. PIC needs to replace damaged or missing ceiling tiles.

Observed multiple containers of dry food products on the prep surface in the front food service area with scoops in the food items with handles touching the food. If stored in food, scoops shall be positioned so the handle is maintained above the food.
