Hallway has damaged walls. Wall has several holes in them. CA: All physical facilities shall be maintained in good repair.

Employee phone on prep table up front. Employee drink on prep table in back. CA: Lockers or other facilities shall be used for storage of employee items.

Observed a colander in the handwashing sink by the cold line, and observed employee pouring something into the sink. CA: A handwashing facility may not be used for purposes other than handwashing. COS: Took the colander out of the sink, and discontinued pouring into the handwashing sink.

Quat sanitizer in 3 compartment sink in the sanitizer compartment was registered as 0 with the facilities' test strips. CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3. of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions, and shall be used as follows: A quaternary ammonium compound solution shall have a minimum temperature of 75°F, have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer's use directions included in the labeling, and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. COS: Re-made during inspection. Registered as the correct concentration.

Observed damaged ceiling tiles in the back room. Observed damaged floor tiles and wall in the hallway. CA: All physical facilities shall be maintained in good repair.

Facility does key drop delivery, but no temps have been recorded on the delivery invoices. There is also no written agreement available in facility to review. CA: The food service records that establishment shall have a written agreement, and show the temp of the products upon receipt of delivery. Facility has 72 hrs to email agreement.

CFSM is expired. CA: A CFSM certificate which has expired shall not be posted in the food service establishment; COS: Taken down during inspection.

Damaged ceiling tiles in the back room. Damaged floor tiles and wall in the hallway. CA: All physical facilities shall be maintained in good repair.

Two spray bottles with sanitizer not labeled. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.P COS: Labeled during inspection.

Spray bottle of sanitizer stored on prep table right next to food, and pans. Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by: 1. Separating the poisonous or toxic materials by spacing or partitioning;P and 2. Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles. This requirement does not apply to equipment and utensil cleaners and sanitizers that are stored in warewashing areas for availability and convenience if the materials are stored to prevent contamination of food, equipment, utensils, linens, and single-service and single-use articles.P COS: Moved during inspection.

Back room has damaged floors, walls and ceiling. The door to the back room has a hole in it. The hallway that leads to the bathroom, and back door has damaged walls, and ceiling tiles that are starting to come up. The back door is also has damaged to the bottom of it, and is not flush with the door frame. This can allow for potential rodents or insects to enter the building. All physical facilities shall be maintained in good repair.

Two mops are still in the mop bucket with the mop head stored down. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Observed spray bottle stored next to the pizza holding hot box/carrier. Poisonous or toxic materials shall be stored so they cannot contaminate equipment; COS: Moved during inspection.

Door to the back, door to the bathroom, and coving in the bathroom damaged. All physical facilities shall be maintained in good repair.

Mop not hung up properly. Mops shall be hung up to allow for air-drying; COS: Moved during inspection.

Observed personal items on pizza boxes. All personal items shall be stored in a designated area. Lockers or other suitable facilities shall be used to store personal belongings; COS: Moved during inspection.

Sanitizer bucket with rag in it has a concentration of 0 ppm. Wiping cloths shall be held in between uses in a chemicals sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). CA: Dumped bucket and refilled with correct solution. COS: Explained rule to the PIC and they are going to ensure the sanitizer is at correct concentration.

Multiple ceiling tiles throughout the facility are damaged. Back door has a hole in it. All physical facilities shall be maintained in good repair.
