No ventilation in bathrooms of rooms 104, 107, 108, 114, 206-208. Toilet, lavatory, and bathing facilities shall be mechanically ventilated. POC: Manager and owner are going to look at system on roof - silent ventilation

After inspecting rooms 115 and then 214 I spoke to the manager about the insects. After speaking with the manager on duty she stated that there are multiple rooms with an infestation of insects, specifically roaches. These rooms include: 103, 105, 106, 108, 109, 110, 111, 112, 114, 115, 119, 202, 207, 208, 214, and 224. The facility has been bombing the rooms and spraying the rooms but the rooms have not been eradicated of the insects. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program.

The facility is doing a partial closure for the rooms that are listed above. The facility has until August 1, 2025 to erratic the rooms of the infestation

Dead roach in the refrigerator in room 212. All appliances shall be kept clean, in good repair, and vermin free

Observed multiple large roaches in the kitchen drawers in room 115 and in room 214. All furniture shall remain in good repair, clean and in good repair

Mattress pad and bed bug cover in room 217 stained. Mattress pad stained in room 208. All bed coverings shall remain clean and maintained in good repair

No ventilation in bathroom in room 208, 207, and 212. Toilet, lavatory and bathing facilities shall be mechanically ventilated

Bedbug on sheet in room 112. Bed coverings shall remain clean and vermin free. COS: Room closed off

No ventilation in majority of rooms. There shall be proper ventilation in all restrooms

Mold in bathroom - 112. Roach on wall - 116. Damaged bathroom walls -126. All floors walls ceilings shall be clean and in good repair

Observed rooms 115 and 207 with stained bed coverings. All bed coverings shall be clean and maintained in good repair

Mattress cover observed with dirt particles on it. All bed covering shall remain clean and in good repair

Paint missing, damaged wall, damaged ceiling tile, and missing coving observed in rooms inspected. All floors, walls, and ceilings shall be maintained clean and in good repair

The gas fired equipment hasn't been inspected since 5/22. All gas-fired equipment shall be inspected at least annually. CA: Inspector coming on monday

Multiple rooms had bed bug covers that had stains on them. All bed covering shall be clean and in good repair

Observed two toilets and a sink counter that needs to be re-caulked. Also observed multiple vent covers, covered in dust. All toilet/bathing facility fixtures shall be kept clean and in good repair
