Observed a live roach in room 206. 511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Have a licensed pest control come to facility as often as necessary

Observed debris under the pull-out couch in rooms 106, 206, and 210. 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Clean under pull-out couches as often as necessary

Observed lamps missing lightbulbs in rooms 210 and 231. 511-6-2-.14(4) - Room Lighting All rooms shall be well lighted. When natural light fails to provide sufficient illumination, evenly distributed artificial light shall be provided to maintain a lighting intensity of not less than ten foot candles at 30" above the floor. Place new lightbulbs in all lamps missing lightbulbs

Observed missing ice buckets liners in room 115. 511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice. Place ice bucket liners in all ice buckets provided to the guests

Observed an old gas-fired equipment inspection on file. 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Have gas-fired equipment inspected by a licensed contractor or local fire authority

Observed several unlabeled chemical spray bottles on the housekeeprs cart. 511-6-2-.17(2) - Housekeeping Carts Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Label all chemical spray bottles

Observed debris behind the nightstand in room 223. Also, observed debris behind the tv stand in room 226. Observed several roaches in the sleeping and restroom areas in in a drawer in room 111. Pest control is performed monthly. Be sure this room receives treatment
