Observed stained sheets in rooms 307 and 102. Also, I observed hair on the bed sheets in rooms 102 and 106. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Replace dirty and stained sheets with fresh clean new linens

Observed debris in the bathroom sink in rooms 319, 106, and 125. Also, observed hair on the bathroom toilet in room 125. There was debris also on the bathroom floor in rooms 307 and 319. 511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Clean bathroom floors, sink,and toilets as often as necessary

Observed black mold-like substance in the laundry room vent. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. Properly treat black-mold in the laundry facility

Observed a pink slime on the first floor ice machine. 511-6-2-.07(8) - Ice Supply In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Clean and sanitize ice machines according to manufacturer's recommendations

Observed live ants in the window sill in room 123. Also, observed dead bugs in the window sill in rooms 227 and 310. 511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Have a licensed contractor come to facility as often as necessary and clean window sill as often as needed

Repair or replace inoperable phones. Also, clean microwaves as often as necessary

Observed a green-algae pool with stagnant water. 511-6-2-.16(1) - Swimming Pool in Compliance with Rules Regulations of the Department and/ county in which govern the design, construction, operation and maintenance of swimming pools or spas shall apply to pools and spas operated in conjunction with a tourist accommodation. Have pool covered or drained to prevent the harborage of mosquitoes

In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations.
