Observed the PIC could not provide the annual gas fired equipment form. 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed one bottle with visible mold. 511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

Rooms 121, 209, 224, and 314 were observed to have stains on the bench cushions. Mini fridges in all rooms were observed to have debris and in need of cleaning

All rooms had stains and hair on the bedding material. Rooms 121 & 209 had stained on the mattresses

Room 103: Wall cover loosely taped to the wall to cover a hole in the wall. Wall must be sealed. Laundry Room: Observed the walls behind the washing machines and mop sink with water damage and paint peeling off the wall

Observed paint chipped and cracked around the shower
