Observed the floor of the reach in freezer next to the fryers with food debris and container lids. Manager was advised to clean the floor of the reach in freezer and remove the lids. Observed mold like substance on the ice chute dispenser of the front soda machine. Please clean to prevent build up.

Assistant manager was observed working the register and proceeded to go into the kitchen to put frozen items into the fryer without hashing hands or donning gloves prior.

Observed single use cups stored at the front counter with the interiors exposed to potential contamination from customers or environmental contaminants from the front door. General manager was advised to move cups to a least exposed area of the front counter or purchase a fully covered cup dispenser for single use cups.

Observed excessive trash and debris build up behind both dumpsters. Team member began to clean trash and debris.

Observed employees without proper hair restraints. All hair must be completely restrained with hairnets and/or caps/hats.

Food debris observed at the bottom of the reach in freezer in the main kitchen.
