Observed three pans of raw chicken liver stored next to raw sliced squash. Three pans of raw pork chop and raw chicken livers were stored next to sliced squash. Two pans of raw fish stored next to raw chicken gizzards. Raw chicken gizzards stored next to pre cooked sausage.

- Food shall be protected from cross contamination by:
(i) Except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display from:
(I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and
(II) Cooked ready-to-eat food; and P
(III) Fruits and vegetables before they are washed; P
(IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food.
(ii) Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by:
(I) Using separate equipment for each type, P or
(II) Arranging each type of food in equipment so that cross contamination of one type with another is prevented, P and
(III) Preparing each type of food at different times or in separate areas; P
(iii) Cleaning and sanitizing equipment and utensils;

Observed old food build up on utensil that were stored in the clean dish storage area. PIC corrected on site by rewashing utensils.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(l) - removing dead or trapped birds, insects, rodents, and other pest (c)
Observed the presence of dead roaches in the cabinet below the salad station.

Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
12A - contamination prevented during food preparation, storage, display

Also observed raw chicken being prepared above a cooler containing cake icing. PIC advised to prepare raw chicken in the raw chicken/meat preparation sink.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed the sanitizing buckets containing quat sanitizer and wiping cloths did not meet the minimum concentration requirements of 200-400 ppm. Sanitizer buckets observed at 50 ppm.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed a leak below the three compartment sink. PIC advised that repairs will be completed by 8/27/25. A service work order must be sent to robert.lewis1@dph.ga.gov by 8/29/25.

17C - physical facilities installed, maintained, and clean

Observed floor tile in bad repair. The floor grout was also missing in some areas. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing
17D - adequate ventilation and lighting; designated areas used

Observed dust build up on the interior of the vent located above the dish washer. PIC will have the exhaust clean and covered by 8/29/25.
14A - in-use utensils: properly stored

Observed spoon handle touching diced onions in the storage bin.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed wet dishes stored top of each other in the storage area. Observed forks and spoons not properly air dried prior to placing in the buffet area for customers. CA- PIC will allow utensils and dishes to properly air dry. 511-6-1.05(10)(a) - Equipment & Utensils, Air-Drying Required (C) (10) Protection of Clean Items.

2.In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon;
3.On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a);
4.In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes;

In a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature control for safety food; or
-
In a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi).

Observed shelves containing old food particles in both walk in coolers. Metal ice scoop located next to the ice bin was cracked.

Observed build up on walk in cooler plastic curtains. PIC advised to clean the curtains once a week or as needed to prevent build up and accumulation.

Observed the dumpster lid is missing on one of the dumpsters. PIC advised that the dumpster is being replaced this week. PIC will email a copy of the service work order to robert.lewis1@dph.ga.gov by 8/29/25.

- Inside the food service establishment if the receptacles and units:
(i) Contain food residue and are not in continuous use; or

- With tight-fitting lids or doors if kept outside the food service establishment.
