Room 110- Observed stains on linen and bedding Room 222- Observed stains on mattress cover and sheets Room 128- Observed heavy stains on mattress pad (Cover). Also observed cigarette burn holes in the comforter.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Room 224- Observed mold build up in the caulking area of the tub. PIC will clean, disinfect, and recaulk by 8/6/25

Room 222- Caulking missing around the tub. PIC will recaulk by 8/5/25

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

Observed the building next door being used to store new ac units and equipment has severe mold build up. PIC was coached last inspection and advised to remove all equipment. The equipment was still present during today's routine inspection. As a result, the location must have the roof leaks repaired mold remediated, ceiling tile changed, and walls painted by 8/15/25. A service work order must be emailed to robert.lewis1@dph.ga.gov. If we do not receive a completed work order by 8/25/25, the hotel permit will be temporarily suspended, and the location will be required to cease operation until the mold is abated

Observed build up in the chute of the ice bin on the first floor. PIC will clean and sanitize by 8/6/25.

511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed the house keeping carts contained unlabeled bottles. PIC corrected on site by writing the common name of chemicals on bottles. Also observed very long hair strands in the clean lined located in the laundry room.

511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label.

Observed no annual fired equipment report for the facility.

All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Room 109: Observed stains and a mold substance on the lap shade and the curtains stained. Food debris inside of microwave. Room 110: Observed rust on lamp on the computer desk and the curtain with holes. Room 221: Observed debris behind the bed and the dresser in bad repair. The sofa chair had holes and the computer chair was in bad repair. Room 119: Observed a ice build-up in the freezer and stains on the sofa chair. Room 218: Observed stains on the sofa chair and the light on the night stand not working

Room 109: Observed the top sheet with holes and stains. Hair and stains on mattress pad. Stains on the box spring cover. Room 110: Hair on the box spring cover. Room 119: Top sheet with stains and holes. Hair on the pillow and mattress cover. Room 218: Observed hair and stains on the mattress pad, comforter stained, and mattress cover stained. Room 221: Observed pillow and pillowcases with stains. The mattress pad had yellow and brown stains. Also hair on the mattress cover

Room 110: Observed the hand sink cracked. Room 119 & 218: Observed a black mold like substance around the tub

Observed building next to the office with an infestation of black mold. Observed several boxes of new AC units stored in the building. PIC advised to not store any TA(Hotel) related items in the building

Room 108- Pillows and pillow covers contained stains. Sheets, mattresses, and mattress cover contained hair and stains. Room 107- Mattress cover contained large stains and hair. Dead bug observed between mattress and boxspring. Room 112- Large yellow stain observed on one of the mattresses. Observed stained towels near the hand sink area. Pillows, mattress cover, and pillow covers stained
