Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed a thick grey accumulation of build up on the interior surface of the dishwasher in the kitchen. Advised PIC to use a decalcifier to clean the dishwasher. All food contact surface should be clean to the sight and touch.

12B - personal cleanliness

Observed a bracelet on employees arm while handling and preparing food. Advised food employee to remove the bracelet (COS).
511-6-1.03(5)(g) - Jewelry (C) (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

Observed the middle door on the pizza make table in bad repair. Also, the screw moving as the door opened and close. PIC advised that a work order is in for the door. The door must be repaired by April 15th. Please send a completed work order by April 15th to sabrina.young@dph.ga.gov.

Observed the shelves in the walk in cooler with a thick amount of build up. Observed the exterior surface of the window on the walk in cooler in need of cleaning due to a thick black accumulation.

17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the ceiling tile in the chemical room in bad repair and stained. Observed the ceiling tile throughout the kitchen with stains. Observed dust build up on the wall in the dry storage area.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(j) - heating, ventilation, air conditioning, system vents, designed to prevent contamination (c)
Observed the HVAC vents throughout the kitchen in need of resealing or replacing due to rust build up.
