Observed black build up on the deflector of the ice bin. PIC will deep clean the ice bin placing focus on the deflector by 03/25/2026.

1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf.

Observed the presence of dead pest in the storage room. PIC corrected on site.

Observed sanitizing solution exceeding the maximum concentration allowed. Observed quat sanitizer exceeded the maximum strength of 400 ppm in the three-compartment sink. The quat sanitizer was observed above 500. The test strips turned blue. Also observed the chlorine sanitizer bottle above 100 ppm. The test strips were bleaching out due to exceeding the maximum concentration. PIC remade the quat sanitizer in the three compartment sink and chlorine in the spray bottle. Both were tested at the proper level once corrected.

511-6-1.07(6)(g) - Chemical Sanitizers, Criteria (P) (g) Chemical Sanitizers, Criteria. Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials applied to food-contact surfaces shall:

1. Meet requirements specified in 40 CFR 180.940 Tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (Food-contact surface sanitizing solutions) P, or

2. Meet the requirements as specified in 40 CFR 180.2020 Pesticide Chemicals Not Requiring a Tolerance or Exemption from Tolerance-Non-food determinations. P

Observed build up on the exterior of the equipment storage containers in the storage room. PIC will properly wash, rinse, and sanitizer the containers. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.

3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed black mold build-up on the ice machine chute.

Observed about (7) reach in coolers without a thermometer to get an accurate ambient temperature.

Observed (1) dead roach inside of the unplugged milk reach in cooler. also several dead bugs around another reach in cooler.

Excessive trash build up in the dumpster area. Liquid leaking from the trash compactor. Trash observed behind the compactor. Several bags of trash observed on the dumpster deck area.

Adequate lighting not provided int he walk in freezer
