4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(l) - mechanical warewashing, hot water sanitization temperatures(p)
Dishwasher not reaching appropriate temperatures. Gauges appear broken but internal temp was only 133.5 F. Will use dishwasher for wash and rinse then dip the dishes in the sanitizer sink in kitchen before allowing to air dry. To have dishwasher serviced and may look at conversion to a chemical sanitation.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(b) - working containers, common name (pf)
2 spray bottles found with no label. 1 was sanitizer and 1 was purple power deep cleaner. COS Manager labeled both containers.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(b) - approved system and cleanable fixtures (p, c)
Hand sink in back fixture is corroded and dinted. Must be smooth and easily cleanable. replace.

No air gar at dishwasher, 3 bay sink, vegetable sink, and ice machine. All have been extended into pipe below. fix to allow full air gap at all required sinks and equipment.

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Ceiling in some areas is rusted. Air vents dirty. Ceiling needs to be cleaned and kept in good repair. Rust is not a smooth easily cleanable surface. Repair.

Wall in storage area is exposed wood and ply wood which is not an approved wall covering as it is not smooth and easily cleanable. Replace with approved materials. Chart from construction manual supplied.
