Observed the food establishment to be serving muffins in the facility. EHS inquired about how the muffins were obtained and the person in charge stated that the muffins were made at home. 511-6-1.03(2)(a)-(n)(p),(q) - Responsibility of PIC (Pf) (a)Operations Not Conducted in Private Home. Food service establishment operations are not conducted in a private home or in a room used as living or sleeping quarters; Pf COS: The PIC voluntarily discarded the muffins from the food service facility. EHS informed the PIC that if the facility would like to cook/bake food in the facility, then they will need to submit plans to the health authority before serving any type of food. The food facility has been approved only for selling Herbalife smoothies.

Observed side doors to not be closed and one of the lids of the dumpster to be in disrepair. 511-6-1.06(5)(n) - Covering Receptacles (C) (n) Covering Receptacles. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered: 2. With tight-fitting lids or doors if kept outside the food service establishment. Note: The facility will need to have these fixed to allow for tight-fitting lids and doors.

Observed heavy trash debris to be in the refuse areas. 511-6-1.06(5)(o),(p) - Using Drain Plugs; Maintaining Refuse Areas (C) (p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Note: The facility will need to have this area cleaned and all trash debris removed.
