13A: .02(1)(d)Facility has the opening inspection report displayed from 03/06/2025 instead of the most recent inspection report that was done on 04/09/2025 resulting in a 100 score. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. (C) Corrective Actions: Facility will print and display the new score from todays date 02/16/2026.

14B: .05(10)(e) 1,2,4Observed several single service items in boxes stored in direct contact with the floor. Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: 1) in a clean, dry location; (C) 2) where they are not exposed to splash, dust, or other contamination; (C) and 3) at least 6 inches above the floor. (C) Corrective Actions: Person in charge relocated all items to the rack in the kitchen located 6 inches off of the floor
