An informal inspection will occur on 6/18/26 to verify that all violations have been corrected. Note: Allergen disclosure requirements discussed with person in charge. Note: Rapid Multi-Surface Disinfectant is not approved for food contact surfaces (including dining tables). Note: Ensure all foods requiring date marking are date marked and kept for 7 days or less. Repeat violations observed today. Three consecutive violations of the same code provision on routine inspections will result in a permit suspension. 2-2D: .07(3)(b)Observed handwashing sinks in front cooking area and men's restroom with no paper towels supplied. (No hot air dryer present in men's restroom).// Each handwashing sink shall be provided with individual, disposable towels,; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that delivers high velocity, pressurized air at ambient temperatures. (Pf) Corrective Actions: Paper towels were supplied during inspection.//

2-2E: .03(6)Person in charge could not provide written procedures for responding to vomit/diarrheal events.// A food establishment shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. (Pf) Corrective Actions: Written procedures were provided to person in charge. Facility already has all supplies and a proper disinfectant on site.//

4-2B: .05(6)(n)Observed chemical warewashing machine actively being used while dispensing less than 50ppm chlorine.// A chemical sanitizer used in a sanitizing solution for a manual or mechanical operations must meet a specified minimum concentration (for chlorine sanitizer it must be 50-100 ppm; for quat ammonia, it must be used according to the manufacturer's specifications). (P) Corrective Actions: Dishes were washed, rinsed, and sanitized in 3 compartment sink.//

4-2B: .05(7)(a)1Observed black mold-like accumulation on interior surfaces of ice machine.SECOND CONSECUTIVE VIOLATION// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf) Corrective Actions: Dishes were washed, rinsed, and sanitized in the three compartment sink.//

10D: .04(8)(b)Observed a carton of heavy cream, 2 containers of sour cream, 2 cartons of buttermilk, and 3 containers of milk held past their printed expiration dates.// Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as "keep refrigerated" and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer's expiration date or the sell-by date. (P) Corrective Actions: Foods were discarded.//

12A: .04(4)(q)Observed boxes of food (cooked chicken wings, spring rolls, shrimp) stored underneath leaking condenser in walk-in freezer. Boxes had ice accumulation on them.// Food shall be protected from contamination by storing the food:
- in a clean dry location; (C)
- where it is not exposed to splash, dust, or other contamination; (C) and
- at least 6 inches (15 cm) above the floor. (C)

15A: .05(6)(a)Observed ice accumulation coming from condenser in walk-in freezer. Observed ice accumulation coming from coils in cooler/freezer for storing beverage glasses. Observed prep cooler on right side of front prep area unable to maintain proper cold holding temperatures. Minimum ambient temperature was 41.6F.// Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)

15B: .05(6)(d),(e)Observed chemical warewashing machine that does not dispense the proper amount of chlorine.// A warewashing machine and its auxiliary components shall be operated in accordance with the machine's data plate and other manufacturer's instructions; and its conveyor speed or automatic cycle times shall be maintained accurately and timed in accordance with the manufacturer's specifications. (C)

16B: .06(2)(r)Observed hot water valve does not work properly at handwashing sink in front cooking area.// A plumbing system shall be repaired according to law; (P) and maintained in good repair. (C)

17D: .07(5)(d)Observed accumulation of debris on vent hood filters above cooking area.// Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. (C)
