2-2B: .03(5)(j)1&2Employee water bottles (opened with twist off tops) stored over prep tables in the rear kitchen area. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). (C) Corrective Actions: The person in charge discarded the water bottles.

2-2D: .06(2)(o)The rear hand sink was observed with black mold like substances in the basin area. The front hand sink was in disrepair (not in working order with the fixture removed). All hand sinks must be in proper working order and be maintained clean to sight & touch. Corrective Actions: Correct by 09/19/2025.

10D: .04(8)(b)Multiple containers of light cream observed with a use by date of 9/6/25. Commercially packaged time/temperature control for safety foods must be used/sold by the use by/best by date printed on the package. Corrective Actions: Items were discarded.

15A: .05(6)(q)1&3The dump sink faucet is leaking and the the drain basin is not emptying adequately. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. (C) Corrective Actions: Correct by 09/19/2025. Ensure that the faucet turns off completely (no dripping) and that the basin drains adequately.

15C: .05(7)(a)2,3The facility requires a detailed cleaning. Grease/grime/food debris observed around the floor/cove base area. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. (C) Corrective Actions: Detail clean the facility by 09/19/2025.
